TB-500 is a common, non-standardized name used for materials related to thymosin beta-4, a naturally occurring 43-amino-acid actin-binding protein studied in cell movement and tissue repair.[1] FDA's 2026 review describes TB-500 free base as a seven-amino-acid synthetic fragment but warns that multiple salts, derivatives, and even different active moieties have been sold under the same name.[15] Product identity therefore cannot be inferred from the label alone.

TB-500 is not an FDA-approved drug. Most proposed benefits are extrapolated from thymosin beta-4 research rather than direct, controlled human trials of a chemically verified TB-500 product.

How It Works

Thymosin beta-4 binds actin monomers and participates in cytoskeletal remodeling involved in cell movement.[1][3] Experimental research also links thymosin beta-4 biology to angiogenesis and cell survival in wound and cardiac-injury models.[4][11]

These mechanisms do not establish that a product sold as TB-500 accelerates healing in people. Evidence about full-length thymosin beta-4 cannot automatically be transferred to a shorter fragment or an ambiguously labeled commercial material.

What the Research Shows

Direct TB-500 evidence is sparse. Animal wound-healing and cardiac-repair studies largely concern thymosin beta-4, not a standardized TB-500 fragment.[4][11] Registered human trials of thymosin beta-4-related investigational products involve specific formulations and indications that are not equivalent to products sold online as TB-500.[7][15]

Claims about faster tendon or ligament recovery or broadly accelerated healing therefore remain unproven extrapolations, not established human outcomes.

Safety & Side Effects

There is no approved TB-500 label and no adequate human database defining adverse reactions, contraindications, interactions, pregnancy risk, or long-term safety. FDA's 2026 briefing identifies chemical-identity and naming inconsistencies as a patient-safety risk and evaluates additional characterization, bioactivity, and safety concerns.[15]

Unapproved-product risks include contamination, incorrect concentration, impurities, and receiving a different active material than the label implies. FDA has taken enforcement action involving unapproved thymosin beta-4 products and products not suitable for safe lay directions.[8][16]

Regulatory Status

TB-500 has no FDA-approved indication or dosage. FDA brought TB-500-related bulk substances to its July 2026 Pharmacy Compounding Advisory Committee; the meeting is part of a review process and is not an approval.[9][14][15]

WADA prohibits thymosin beta-4 and its derivatives, including TB-500, under the Prohibited List.[10] Anti-doping status is separate from clinical evidence and applies even if efficacy remains unproven.

Anti-Doping and Sports Medicine Considerations

TB-500's injury-recovery marketing makes its anti-doping status especially relevant. Prohibition does not prove that a substance is clinically effective; it is a separate rules-based determination.[10]

Athletes subject to anti-doping rules should verify the current Prohibited List and use evidence-based sports-medicine and rehabilitation care rather than relying on research-chemical claims.
