How it works

LL-37 is not a synthetic laboratory invention. It is a naturally occurring human peptide, produced by cleaving a larger precursor protein called hCAP-18, which is encoded by the CAMP gene.[1] Neutrophils, skin cells, and other epithelial tissue produce it as part of the body's innate immune system, making LL-37 fundamentally different from engineered growth-hormone-style research peptides.

Its name reflects its structure: it begins with two leucines and contains 37 amino acids total.[2] LL-37 folds into an amphipathic alpha-helix, with one face attracted to fats and the other to water, a shape that helps it interact with microbial cell membranes and immune cells.

Benefits & side effects

LL-37's role in normal human biology is well established: it is a documented, naturally produced component of innate immunity at skin, airway, and gut surfaces.[3] Laboratory studies show activity against a range of bacteria, fungi, and some viruses under controlled conditions, though lab activity does not automatically predict effectiveness in a real infection.[5]

Human clinical evidence is limited but not absent. A randomized, placebo-controlled trial tested topical LL-37 at 0.5 mg/mL and 1.6 mg/mL in patients with hard-to-heal venous leg ulcers and reported improved wound healing with acceptable tolerability.[7] That is a genuine, if narrow, positive human signal — but it is a single trial in one wound type using one topical formulation, not evidence supporting broader antimicrobial, immune, or dermatologic use.
