How it works

SS-31 is a small, synthetic mitochondria-targeted tetrapeptide (four amino acids) that is better known by its clinical development name, elamipretide, along with earlier designations MTP-131 and Bendavia.[1] It belongs to the Szeto-Schiller family of aromatic-cationic peptides, originally designed to concentrate specifically within mitochondria rather than act at a cell-surface receptor.

Unlike many of the compounds covered on this site, elamipretide is a genuine investigational drug that has progressed into human clinical trials, including Phase 2 and Phase 3 studies, for specific rare mitochondrial diseases. That places it well ahead of unapproved research chemicals in terms of regulatory scrutiny, even though it is not yet a broadly FDA-approved medicine.

Benefits & side effects

Preclinical evidence is broad: mouse and cellular studies report improved mitochondrial respiration, reduced oxidative damage, and better skeletal-muscle bioenergetics in aged or injured tissue models.[4] Human evidence exists but is indication-specific and mixed. Clinical trials have evaluated elamipretide in primary mitochondrial myopathy (measuring walking distance and fatigue), Barth syndrome, heart failure, and ischemia-reperfusion settings, with results that vary by trial and endpoint rather than uniformly confirming benefit.[5]

The key distinction researchers draw is between biomarker improvement — such as better mitochondrial respiration readings — and clinical outcomes that matter to patients, like function, symptoms, or survival. For SS-31, the biomarker story is more consistent than the clinical-outcome story, and both should be read as disease-specific rather than generalizable to healthy adults seeking energy or anti-aging effects.
