What It Is
Human chorionic gonadotropin (hCG) is a two-subunit glycoprotein hormone produced mainly by placental tissue during pregnancy, and it is the hormone that pregnancy tests detect in blood or urine.[1] Its levels typically rise quickly in early pregnancy, peak around the end of the first trimester, then decline and plateau.
Beyond pregnancy testing, hCG is also an FDA-approved prescription medication for specific reproductive indications: as a "trigger" injection to complete final egg maturation and time ovulation in assisted reproduction, and in select male hypogonadotropic conditions to stimulate testicular testosterone and sperm production.[2] It is additionally used clinically as a tumor marker for certain cancers, including some testicular and gestational trophoblastic diseases.
How It Works
hCG is structurally and functionally related to luteinizing hormone (LH), and both act on the same receptor, the LH/hCG receptor (LHCGR).[3] That shared receptor is why hCG can stand in for the body's own LH surge in fertility medicine, triggering the final steps of egg maturation, or in men, stimulating testicular Leydig cells to produce testosterone.
hCG acts longer in the body than LH partly because it is more heavily glycosylated, slowing its clearance. In natural pregnancy, this same receptor activity supports progesterone production from the corpus luteum, maintaining the uterine environment until the placenta takes over hormone production.
What the Research Shows
For its approved indications, hCG's evidence base is well established: decades of use in assisted reproduction to trigger ovulation, and in male hypogonadotropic hypogonadism to stimulate endogenous testosterone and sperm production, are reflected directly in FDA drug labeling.[2] As a pregnancy and tumor-marker test, quantitative hCG measurement is a standard clinical tool, though a single reading is generally less informative than a trend followed over serial measurements.
The picture is completely different for hCG as a weight-loss aid. Systematic reviews and clinical trials have found no meaningful weight-loss benefit from hCG beyond what calorie restriction alone produces.[4] This split — strong evidence for reproductive and diagnostic uses, essentially none for fat loss — is the single most important thing to understand about hCG research.
Safety & Side Effects
Prescription hCG used in fertility treatment carries real, medically significant risks that require clinical monitoring, most notably ovarian hyperstimulation syndrome (OHSS), a potentially serious complication of ovulation-triggering protocols.[5] Because of this, hCG therapy is not a self-directed wellness intervention — it requires supervised dosing, labs, and follow-up.

Regulatory Status
hCG is FDA-approved as a prescription drug, but strictly for defined indications: assisted-reproduction ovulation triggering, and specific male hypogonadotropic conditions involving testosterone and sperm production.[2] Products such as Pregnyl and Novarel carry full FDA prescribing labels covering dosing, warnings, and monitoring for these approved uses.
The picture is entirely different for hCG marketed as a weight-loss aid. The FDA has explicitly warned consumers to avoid hCG diet products — drops, pellets, and sprays sold for fat loss — stating that hCG has not been shown to be effective for weight loss beyond calorie restriction, and that many such products are illegally marketed.[6] The same molecule is therefore simultaneously a legitimate, FDA-approved reproductive medicine and the subject of an explicit FDA weight-loss warning, depending entirely on how and why it is being used.
The hCG Weight-Loss-Drops Myth
The "hCG diet" — very-low-calorie eating paired with hCG drops, pellets, or sprays marketed as appetite suppressants or fat-mobilizers — is one of the most persistent misuses of this hormone. Controlled trials and systematic reviews have consistently found that any weight lost on these protocols tracks with the severe calorie restriction itself, not with hCG.[4]
Many over-the-counter "hCG drops" do not even contain meaningful amounts of real hCG, since authentic hCG is a peptide hormone that is not orally bioavailable the way drops or sprays imply. Combined with the FDA's explicit consumer warning against these products, the weight-loss-drops narrative is a marketing myth layered on top of a real, but unrelated, prescription hormone.
