What hCG is
Human chorionic gonadotropin (hCG) is a two-subunit glycoprotein hormone produced by placental tissue. It activates the luteinizing-hormone/hCG receptor, but the name covers more than one drug product: urine-derived chorionic gonadotropin and recombinant choriogonadotropin alfa have distinct manufacturing, formulations, routes, labels, and strengths.[1][2]
A label such as "hCG 5,000 IU" on an online vial does not establish which product it is, whether the stated activity is accurate, or whether it meets the identity, purity, sterility, and manufacturing controls of an approved medicine.
What is established in humans
Specific prescription hCG products have established reproductive indications. Ovidrel, a recombinant choriogonadotropin alfa product, is labeled for final follicular maturation and early luteinization in appropriately pretreated infertile women in assisted reproduction, and for ovulation induction and pregnancy in certain anovulatory infertile patients.[1] Current urine-derived products such as Pregnyl have their own product-specific labeling, including selected female infertility indications and selected male hormonal conditions.[2]
This is grade A evidence only for the exact approved products and labeled clinical settings. It does not establish hCG as a general wellness, testosterone-recovery, bodybuilding, or fertility-preservation treatment, and it does not validate an unapproved research vial.
Weight-loss claims are contradicted by evidence
FDA states that hCG is not approved for weight loss and that there is no substantial evidence it increases weight loss beyond calorie restriction, redistributes fat, or reduces hunger.[3] A criteria-based meta-analysis of controlled trials likewise found no scientific evidence that hCG improves weight loss, fat distribution, hunger, or well-being in the so-called Simeons diet.[4]
Products sold as oral drops, pellets, or sprays for weight loss are not equivalent to approved injectable prescription products. The very-low-calorie diets commonly paired with these products can themselves cause gallstones, electrolyte disturbances, arrhythmias, and other serious harm.[3]
Safety depends on product and setting
Approved reproductive use requires diagnosis, laboratory and ultrasound monitoring, and product-specific instructions. Labelled serious risks include ovarian hyperstimulation, ovarian cyst rupture with bleeding, multiple pregnancy, and thromboembolic events; contraindications and risks differ between products and populations.[1][2]
FDA has also received reports of pulmonary embolism, cardiovascular and cerebrovascular events, depression, cardiac arrest, and death when hCG injections were used for weight loss; reports do not by themselves prove causation, but they reinforce why an unapproved indication and severe diet should not be treated as benign.[3]
Approval does not transfer to research or compounded vials
FDA approval belongs to a finished drug product with a defined manufacturer, formulation, strength, route, indication, and labeling. It does not attach to every substance sold under the name hCG. Compounded drugs are not FDA-approved, and FDA does not verify their safety, effectiveness, or quality before marketing; a compounded product is also not an FDA-approved generic.[5]
Any concentration or administration information must therefore come from the exact prescribed product label and the treating fertility or endocrine team—not from a generic hCG chart.
